Vacation Physical Activity Log
The last day before Christmas vacation I hand out an Activity Log for the students to fill out during their vacation. I have three columns. One column for every day of the vacation, one for the type of physical activity the student partook in, and one for the parent/guardian signature. The parent/guardian signature is to ensure as best we can the student actually performs the activity. It is also hoped that the parents and/or guardians get involved in the activities with their child as well. The students have to return the signed logs to me upon return from vacation. If the student chooses to exercise over vacation at least 8 days or more, the student gets to sign a bulletin board paper that says "I chose to be active during vacation". Some activities I mention that are acceptable: curl-ups, push-ups, jogging, walking, jumping rope, bike riding, stretching, sport practices, tag games, one on one basketball, roller blading, roller skating, shoveling snow, etc.
