Name of Activity: Football Passing Self Check Assessment

Purpose of Activity: This assessment is designed as a "Self-Check" to allow students to chart personal progress made during a single lesson and record successful throwing attempts using a logical progression of tasks from easy to hard.

Suggested Grade Level: 5-7

Materials Needed: Self Check Football Passing Assessment Sheet

Description of Idea

This assessment provides an opportunity for students to work alone and test their football passing skill level. Each task involves throwing a football to the playground fence and trying to hit the hula hoops that are hanging at different levels. The tasks are ordered from easy to hard.

The following elements are altered as students move through this five-task worksheet:

1. Throwing force (light, moderate, strong)
2. Distance (near, moderate, far)
3. Thrower position (stationary, moving forward, moving backwards or sideways)
4. Level of trajectory (medium, high or low)

Students perform each task 10 times and record how many times they were successful (using the "things to look for" or "cues" in the space provided. They are responsible for retrieving their own football each time while respecting the space and work of others.
