Name of Best Practice: Limited Participation Letter to Parents
Rationale/Purpose of Event: Throughout the school year students will periodically encounter illness or injury which will inhibit their full participation in physical education. In order to provide a safe physical education experience for such children, it is necesary for teachers to be informed of their physical condition. The form letter offered below is one that the physical education teacher can supply to parents, who in turn take to a physician. Once the form is completed by the physician and returned to the teacher, appropriate modifications can be taken.
Suggested Grade Level: 6-8
Materials Needed: The handout below.
